- ✪ ✪ ✪ ✪ ✪The doctor was nice but I have been try to get my FMLA forms filled out and it's been taking too long. The first time they filled it out with the wrong dates and it had other discrepancies. I've had to get two extensions on it because the office doesn't seem too see the urgency in the matter. It has been over a week and still no forms and everytime I try to see the doctor he isn't in the office. I have four days left and they have not gave me the slightest clue if anything has been corrected or even looked at.
